Informed medical consent

What it means to receive care through the independent clinicians and laboratories you reach via Activated, the benefits and risks of telehealth, and the rights you keep. You acknowledge this consent when you create your account, and it applies whenever you use a clinical service through Activated.

What this consent covers

Activated, operated by Activated Technologies LLC d/b/a Activated, is a technology platform. The clinical services you can reach through it, meaning laboratory testing, clinician review of your results and requests, and any prescriptions, are delivered by independent licensed professionals, CLIA-certified laboratories, and licensed pharmacies ("care partners"), not by Activated itself.

This consent explains how that care works, primarily when it is delivered remotely ("telehealth"), so you can make an informed decision before using it.

What telehealth is

Telehealth means receiving healthcare services from a clinician who is not in the room with you, using electronic communication, for example, a clinician reviewing your lab results, questionnaire answers, and health history to decide whether a test or treatment is appropriate, or messaging and video visits.

The clinician may practice in a different location than you and will hold the licenses their role and your state require.

Benefits of telehealth

Easier and faster access to testing and clinical review, without travel or waiting rooms.

Your results, history, and goals are available to the reviewing clinician in one place, which supports more informed decisions.

Care can be delivered where and when it works for you, including asynchronous review that does not require an appointment.

Risks and limitations of telehealth

A remote clinician cannot perform a hands-on physical exam, which in some situations can mean a delay in diagnosis or the need to refer you to in-person care.

Care decisions rely on the information you provide. If it is incomplete or inaccurate, the assessment can be wrong. Technology can also fail. Connections drop and messages can be delayed.

As with any health service, security safeguards can fail despite our and our partners' protections. In rare cases delays or errors could result in harm. A clinician may also decline to provide a service where telehealth is not appropriate.

Telehealth is not for emergencies. If you think you are having a medical emergency, call 911 or go to the nearest emergency room.

Laboratory testing

Sample collection is performed at partner draw sites or by mobile phlebotomists, and analysis is performed by CLIA-certified laboratories. A licensed clinician orders the tests where the law requires it.

Lab results on their own are not a diagnosis. Out-of-range values are common and interpretation depends on your full picture. The app's explanations are educational, and a clinician should be involved in any medical decision.

You may be contacted by a clinician if a result requires urgent follow-up.

Your rights and acknowledgements

You can withhold or withdraw this consent at any time by stopping your use of clinical services or emailing us. It will not change the care you have already received, and you keep access to the educational parts of the app.

You have the right to ask questions about any service before you use it, to know the clinician's identity and licensure, and to receive a copy of your records.

You acknowledge that: the results of any care cannot be guaranteed; you are responsible for providing accurate, complete information; care partners, not Activated. Make all clinical decisions; and no doctor-patient relationship exists with Activated itself.

Your health information is handled as described in our Privacy Policy, and shared with care partners only as needed to deliver the services you request.

Complaints and questions

Questions and concerns about a clinical service can be sent to support@activated.app. We will route them to the right care partner and follow up.

You may also contact the medical board of your state directly with a complaint about a clinician; each state board publishes its process online.

Acceptance

By checking the consent box when you create your account (or by using a clinical service through Activated), you confirm that you have read and understood this consent, that you accept the risks and limitations described here, and that you consent to receive services from Activated's care partners, including through telehealth.
